Online Forms and Guides

​​​​​​​​​​​​​​To assist you with the court process, Court Services has made all forms available online in fillable PDF format.

You may need install a free version of Adobe Acrobat Reader to complete the fillable PDF files on your PC (preferred option) or download the Adobe App if using a mobile device.

Option 1 - Download the form and save locally, complete the document (fields outlined in RED are mandatory) and click on the Email Form button located in the top right of the form to send via email.

Option 2 - Download the form and save locally, complete the document (fields outlined in RED are mandatory) ​email as an attachment to the appropriate email address within the form details sections.

Option 3 - Download the form, complete the document and send by mail to the address indicated on the back of your ticket.​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​

You do not have to attend in person to request a trial. Requests for trial may be made by e-mail by completing a Notice of Intention to Appear (NIA) form and sending the completed form to requestatrial@brampton.ca​; or by mail to the address on the back of your Offence Notice.

Schedule an Early Resolution meeting with the prosecutor to discuss the possible resolution of the charge. All Early Resolution meetings with the prosecutor will be available via telephone, regardless of a person’s distance from the courthouse.​

To ask the court for more time to pay the fine as indicated on your ticket on a Provincial Offence Matter. If you need more space to state your reasons for extending the time to pay, you may also complete and attach additional page(s).

To apply for a Reopening of a conviction from a Trial or Early Resolution hearing, complete the application below and

​To request the adjournment of first trial date on an Offence Notice. Ple​ase note that your original trial date is not changed until you receive a new date from our office.​ . Complete the application form and submit by email to requestatrial@brampton.ca​ at least five (5) business days prior to the original trial date. Only one adjournment request is permitted.

To request a photocopy of court file, complete this form and submit by email to provoff@brampton.ca​ or deliver by mail to the court office indicated on your ticket or w​here your court hearing was held. There are regulated fees for photocopies ($2.00 per page for plain copies and $3.50 per page for certified copies)

To request the adjournment of first Early Resolution date on an Offence Notice. Please note that your original Early Resolution date is not changed until you receive a new date from our office.
